Please adhere to the following guidelines when submitting custom definitions for testing:
* The <code>id </code> for each Form should be unique and follow existing naming conventions. This <code>id</code> is shown as the {{man label|Code}} to use to access the form.* Attribute names should be consistent with other form definitions for the country/region.
